Reduction for Disabled Facilities Form

The information that you provide on this form will be used to maintain Council Tax and any related Benefit records. The information may be compared with that held by other Council departments, or other organisations, for the purposes of preventing or detecting fraud and in accordance with the Authority's notification under the Data Protection Act 1998.

If you wish to claim money off your Council Tax bill because your home has been adapted for a disabled person, please fill in this form.

Grounds for Application


Sufficient floor space for someone who uses a wheelchair within the home.
A room - not a bathroom or kitchen or lavatory - which is used mainly by and is needed to meet the special needs of the disabled person.
An additional bathroom or kitchen which is required to meet the needs of the disabled person.
