National Minimum Data Set (NMDS) Questionnaire

To get funded training, or to use the reporting facilities in NMDS-SC online, you need to complete or update an NMDS questionnaire on the Skills for Care website.

If you have already completed an NMDS questionnaire and want to update it, log in to your account with your username or password. If you have forgotten your username or password, please call the Skills for Care helpline on 0845 8730129.

We recommend that you check your questionnaire every two months, to make sure it is fully up to date.

If you have not yet completed an NMDS questionnaire, you will need to set up an account first and get your own username and password. The leaflet below, a Quick Start Guide for establishments with no NMDS-SC number, explains how to do this.

When you have completed the organisation part of the NMDS questionnaire, you will be given a unique registration number. Please use this number when you contact Birmingham Care Development Agency (bcda), especially when requesting training so that it can be used to claim the cost of the training from Skills for Care.

If you don't have access to the internet and would like to complete the NMDS questionnaire on paper, please contact bcda by phone on 0121 303 0887 or by email.

If you would like bcda can act as an agent on your behalf, please ask bcda for a Data Protection agreement.
