Services for people with a physical disability
Introduction
The physical disability service give advice carries out assessments and support people with a physical disability and their carers for adults between 18 and 64.
Assessments
Community Care Assessments and reviews are conducted by social workers and review officers from the above teams. Our role is to provide advice, assessment and care planning for those with identified complex care needs to enable them to maximise their independence through support planning.
